Communicates with clients, families, personnel and other support agencies in a friendly and courteous manner.
Ability to lift/move 50 pounds.

RESPONSIBILITIES/DUTIES Develop, and revise as necessary, an Activity Plan for each participant that takes in consideration the needs, abilities, and interests of the participants.
Research new ideas for activities and programs beneficial to the participants that are currently attending the program.
Generate and post a monthly activity calendar.
Individualize activities as needed for participants with special needs.
Conduct daily meaningful activities for participants.
Attend and contribute to interdisciplinary care conferences for each participant every 6 months.
Complete and maintain documentation required for participants.
Arrange for outside entertainment to come to facility.
Arrange for and accompany participants on outings in the community.
All job responsibilities will be carried out in accordance with Heritage Manor Adult Day policies and compliance/ethics guidelines.
Assist with participants care needs.
Complete all other duties as assigned.
